Court forms program
Free New Jersey
court forms.
Forma Pauperis is Jacobrakai Foundation's free court-forms program for people handling a New Jersey court matter on their own. It gathers 1,912 official forms in one place and helps you fill them in, with no account, no fee, and no advertising.
Why the Foundation runs it
The Foundation works to remove paperwork as a barrier to rights people already have. In court, that barrier is often the form itself: hard to find, hard to read, and easy to fill in wrong. Forma Pauperis puts the official forms where people can find them and walks through them one question at a time.
The name is the Latin term courts use for a person allowed to proceed without paying fees because they cannot afford them. The court's fee-waiver forms are part of the collection.
How it works
- Every form is the official PDF, downloaded unaltered from its government source.
- Forms with fillable fields become step-by-step interviews. The questions are the form's own labels, in the form's own order.
- Documents without usable fields, such as scans and booklets, come as the blank official form plus a typed answer sheet to copy by hand.
- When you finish, you download a PDF. The site does not file anything with a court; you file it yourself.
Your privacy
You never create an account or give a name, email, or phone number to use the site. Answers are stored encrypted only while you work and are deleted after 30 minutes of inactivity, along with any PDF you generated. The site has no analytics, no ad trackers, and no third-party code.
A court form still asks who you are, and your browser keeps its own history and downloads. Read How this works & your privacy before you start, especially if someone else might check your device.
Legal information, not legal advice
Forma Pauperis helps you complete a form. It does not tell you which form to file, whether you have a case, or what to write, and using it does not create an attorney–client relationship. The Foundation is not a law firm and is not affiliated with the New Jersey Judiciary.
If you can, have a lawyer or your county Legal Services of New Jersey office review a form before you file it.

The forms and interviews run on the program's own site.
